The Intensive Case Management (ICM) program offers intensive case management services to individuals with complex mental health needs/concerns which seriously affect the individual’s life. Intensive case management promotes independence and quality of life through the coordination of appropriate services and the provision of constant and on-going support, as needed by the client-member. Case management services at RCS are client-member-centred – case managers assist individuals in setting personal recovery goals and arrange/coordinate the support required to achieve those goals, involving client-member support teams (e.g. family, friends or agencies) where appropriate. The ICM team has partnerships with CAMH and St. Joseph’s Family Health Team and also receives referrals from other sources.
Case managers meet regularly with client-members in the ICM program to:
- Complete assessments to identify (and later reassess) needs, strengths and recovery goals;
- Develop a plan for services which incorporates client-member goals and values, and focuses on priority areas of need (such as housing, education, social relationships, employment, daytime activities and health-related issues);
- Connect client-members to outside resources and services through referrals and/or community engagement;
- Facilitate and coordinate access to services, including consulting other individuals and agencies within a person’s circle of care (with the consent of the client-member);
- Provide supportive counselling in order to assist client-members in achieving personal recovery goals; and
- Advocate for client-member rights both on an individual and systemic level.
